This Telehealth Informed Consent describes the use of telehealth services offered through LighterRx in partnership with licensed providers and a licensed U.S. compounding pharmacy.

What is telehealth?

Telehealth involves the use of secure electronic communications to enable providers to evaluate, diagnose, and treat patients remotely. Your care may be delivered without an in-person visit.

Benefits and limitations

Telehealth can improve access and convenience. However, a remote evaluation has limitations, including the inability to perform a hands-on physical exam. Your provider will determine whether telehealth is appropriate for your needs and may recommend in-person care.

Your responsibilities

You agree to provide accurate and complete information about your health. Incomplete or inaccurate information can affect the safety and quality of your care.

Prescriptions

A prescription is issued only if a licensed provider determines it is clinically appropriate. Treatment is not guaranteed. Compounded medications are not FDA-approved.

Emergencies

Telehealth is not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency room.
